This is the online preliminary application for Family Adoption Consultants. The fee for submitting this application is $200. This preliminary application fee is non-refundable and non-applicable to any other program or service fee of Family Adoption Consultants.  Preliminary applications expire if not acted upon within six (6) months of date filed. Please print and mail this form along with your check, to one of the FAC offices. You can find our address under the CONTACT US tab.
